LIABILITY & MEDIA RELEASE STATEMENT

LIABILITY WAIVER STATEMENT (Please read the following statement carefully)

I hereby acknowledge that I registered my child(ren) in the Al-Kahf Robotics program. I understand that the program will be responsible for the care of my child(ren) during the length of the program. While close supervision, reasonable effort and precautions will be made to minimize risk and ensure the safety of my child(ren), I acknowledge that there are certain risks associated with my child(ren) taking part in the program and its activities, including but not limited to:

Physical injuries: Active games can involve running, jumping, climbing, and other physical activities that carry a risk of injury. Children may fall, collide with others, or strain their muscles while engaging in these activities.

Accidental falls: While participating in or navigating program activities, children may trip, slip, or lose their balance, leading to falls and potential injuries.

Allergic reactions: program activities involve using various materials, such as paints, glues, and fabrics, which can cause allergic reactions in some children.

Choking hazards: program activities may involve small objects, beads, or craft supplies that pose a choking hazard.

I release and hold harmless the Islamic Information Society of Calgary (IISC), its officers, directors, agents, contractors, employees, volunteers, members and representatives from any and all actions or causes of action of any nature for personal injury or property damage of any kind arising in any way from my child(ren)’s participation in the program. I acknowledge the nature of the activities and the provisions for my child(ren)’s involvement and consent to my child(ren)’s participation in the program. I further acknowledge that this Release is binding upon my heirs, successors or assigns, that I have read the previous and understand its significance, and that I have executed this document voluntarily.

MEDIA RELEASE STATEMENT (Please read the following statement carefully)

I, hereby, grant permission to the Islamic Information Society of Calgary (IISC) to use my and/or my child(ren)’s photo, video recording or other items, from now on referred to as “Materials,” taken during the program. I hereby release the Islamic Information Society of Calgary (IISC), its officers, directors, agents, contractors, employees, volunteers, and members from all claims and demands arising out of or in connection with any use of the “Materials,” including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights. I acknowledge and agree that no sums will be due to me due to the use of the “Materials” or any rights therein.

I acknowledge that my child(ren)’s is/are under 18 years old and lacks the legal capacity to enter into binding agreements. Accordingly, I have read this Release and consent to my child(ren)’s inclusion in the “Materials” and will not contest the rights granted in this Release.
